New Patient Packet
New patients need to complete these forms prior to their first appointment. You can print it out and complete it at home or we have printed copies available when you check-in.
Vanderbilt Forms
Prior to the consultation appointment for an adhd dignosis we need to have the Vanderbilt forms completed. The forms should be completed by the parents as well as the teacher(s). This appointment is longer than a regular appointment and must be completed separately from the well-child visit.

Medical Records Release
If you are joining Carmel Pediatrics, please obtain health records and vaccine information from previous health care providers before your first appointment.

If you are leaving Carmel Pediatrics, we are sorry to see you go! Please note there is a $15 labor fee to transfer records.
