New Patient Packet
New patients need to complete these forms prior to their first appointment. You can print it out and complete it at home or we have printed copies available when you check-in.
Sports Forms
The IHSAA health form can be completed without an additional appointment if your child has had a Well-Child visit within the last year. Complete the authorization form and the parents section of the IHSAA form and email the forms to info@carmelpediatrics.com. Once the completed form is received we will contact you for payment.
Childcare Forms
Childcare facilities frequently require the state childcare form for enrollment. They may also request the feeding form and/or the medication form. Please check with the facility to make sure we get them all completed at the same time. We need an authorization form completed. Once you have the parent portion of the childcare form(s) completed, email the forms to info@carmelpediatrics.com We will contact you for payment after we receive the forms.
Other Health Forms
Many camps require a specific health form completed. These do not require a separate appointment if your child has completed a Well-Child visit within the last year. Please email the completed forms to info@carmelpediatrics.com. Once we receive the completed forms we will contact you for payment.
